Public Health Nurse to collect immunization records on June 24th, from 8- 8:25 am

Children who attend school in Ontario are required by law to be immunized against vaccine-preventable diseases or have a valid exemption. To make sure students are safe from these diseases, Ottawa Public Health (OPH) is required to keep information on the immunization status of every child attending school in our city. Students who do not have up to date immunization records may be prevented from attending school. 

 

To assist parents in providing OPH with their child’s immunization records, a public health nurse will be available to assist with updates at St. Michael School, from 8 a.m. – 8: 25 a.m.  on Wednesday, June 24th.  Please bring your child’s immunization record or a photocopy of your child’s immunization record with you.
